The appearance of a child brings to the life of the family not only new concerns for caring for the baby, but also many other concerns. We live in a state, so a family event must be recorded in government agencies, and the little man must get all the documents in the near future. It is necessary to draw up the documents necessary for a newborn child during the first month of life. Our "cheat sheet" will help you not to get confused in this procedure and to meet the deadlines.

Documents from the hospital

In fact, the baby receives the first documents already upon discharge from the hospital. They confirm the fact of his birth and serve as the basis for the issuance of other documents, and the mother is given the opportunity to be observed in the antenatal clinic after delivery.

When leaving the hospital, the mother has in her arms:

1. Part of the exchange card. With an exchange card, a woman is admitted to the hospital. Part of this card is filled in at the hospital and issued to the mother:

  • The 2nd sheet, which contains information, contains information about the course of labor and the postpartum period. It must be submitted to the antenatal clinic where the pregnancy was managed within 10 days after discharge.
  • 3rd sheet, which contains information about the newborn baby: his parameters at birth and at the time of discharge, assessment of his condition, information about the vaccinations made, about the application to the breast. This document is required for the children's clinic. It is attached to the child's card. It is best to pass on the sheet with information about the baby to the nurse or doctor who takes care of the newborn.

It is worth noting that both of these documents can be in another form, not as part of an exchange card. The fact is that the "exchange" does not have a strict form, some cards may not contain the necessary sheets at all, so all the necessary information from the hospital is transferred on other forms.

2. Certificate of the birth of a child (certificate-statement, medical certificate of birth). Without this certificate, further paperwork is impossible, it is she who confirms the fact of the birth of a child by a specific woman. It records where, when and by whom the baby was born, what gender he is, who of the staff gave birth.

3. The third coupon of the generic certificate and its annexes. The coupon itself remains at home, and two supplements to it (3-1 and 3-2), along with information about the newborn, are transferred to the children's clinic. This is necessary to compensate for the cost of medical supervision of the child in the first year.

Birth certificate

Obtaining a birth certificate for a baby is possible in any department of the registry office: at least at the place of registration, at least at the place of actual residence, if the registration and current place of residence do not coincide. To do this, mom or dad must come to this institution with the necessary documents within the first month after birth. The following documents must be submitted to the registry office to register a newborn:

  1. Birth certificate of the child, which was issued at the hospital.
  2. Passports of both parents (or only one of them).
  3. A marriage document (if the marriage is official).

To obtain a birth certificate, you will need to write an application. The registry office will help you to do this. The child's birth certificate contains data on the date, place of birth, and parents. The surname is assigned by the surname of one of the parents, and the name is assigned according to the words of the parents. Also, at the request of the parents, the nationality of the baby can be indicated there.

A birth certificate is issued quickly: usually it is ready already on the day of application. Together with the birth certificate, the registry office issues a birth certificate. This document has a unified form F-24 and is needed to register a one-time allowance for the birth of a child.

Registration and citizenship

A newborn child can be registered only where the parents are registered or one of them, if they are registered at different addresses. This is done by the FMS. To obtain registration, you must submit the following list of documents:

  1. A statement by either parent about the need to register the newborn at the place of registration of the mother (father).
  2. Extracts from the personal accounts and house books of the EIRTs and the passport office from the place of residence of the father or mother.
  3. A certificate from another parent, certifying that the baby is not registered with him (in case the parents are registered in different places).
  4. Baby's birth certificate (original and copy).
  5. Parents' passports (originals and their copies).
  6. Marriage document (if registered).
  7. A statement from the second parent, in which he confirms that he is not against the registration of the child.

For registration of registration, the presence of the parent with whom the child will be registered is mandatory. In contrast to the registration of a birth certificate, registration takes more time and requires more trouble. As for the timing, there are contradictions on this issue in the laws: as a newcomer to the place of registration, the child must be registered within 7 days from the date of birth, however, without a birth certificate, registration at the place of residence is impossible, and for registration of a birth certificate given a month. In addition, there is another rule of law: if a person plans to stay in the area for less than 90 days, he does not need registration, therefore, conditionally, the child can live without a registration for the first 90 days.

Citizenship registration is also done at the department of the FMS, so that the acquisition of citizenship and registration of registration usually take place simultaneously.

OMS policy

This paper is needed so that the child can receive the free medical care due to him. All citizens of the Russian Federation have the right to compulsory health insurance. It is also possible to independently choose an insurance company. The compulsory medical insurance policy is issued at the actual place of residence, it is in no way tied to registration.

In order to order a compulsory medical insurance policy for a newborn, you need to contact the office of the insurance company (usually they are available right on the territory of the clinic) and submit the documents:

  1. Proof of the child's identity, that is, his birth certificate.
  2. Passport of one of the parents (the one who submits the documents).

Mom or dad will need to fill out an application form for an OMI policy. A permanent policy is made for about a month, but after the documents are accepted for registration of the policy, a temporary certificate is issued.

This completes the receipt of documents for the little man. It is not worth forgetting or ignoring the execution of any documents, since administrative fines are threatened for violation of the law. It is likely that very soon the heap of documents will be replaced by one plastic card, for the registration of which it will be enough to submit a package of documents in one window. 1. Birth certificate

Contact the registry office. Documents:

  • statement;

2. Registration at the place of residence of the parents

4. Compulsory health insurance policy

Contact the polyclinic. Documents:

5. Lump-sum allowance for the birth of a child (12 405 rubles) + allowance for registration in early pregnancy (465.20 rubles)

Working parents need to get from the accounting department:

6. Monthly allowance for caring for a child up to 1.5 years old

The monthly allowance is assigned to the one who directly sits with the child. It is paid at the rate of 40% of the average monthly salary. Contact one of the parents at work or school. Documents:

  • application for the provision of parental leave and the appointment of benefits and compensation;
  • birth certificate;
  • certificate from the spouse's place of work that he does not receive this allowance;
